Please contact your insurance carrier to determine your specific benefits including any deductibles, co-insurance, and out-of-pocket expenses. Please verify that our physician is an in-network provider on your plan. Be advised that our clinic bills according to the time spent with the medical doctor or other health care provider and includes the amount of time spent documenting in the medical record. If your insurance does not cover for extended visits (55) minutes or longer, you may receive a bill.

Additional Fees:
You will be charged a fee for all forms requested to be filled out by your physician, such as those for FMLA, day care, pre-employment, life insurance, family medical leave, and other required forms. The fee is $125 as it takes more time and detail to ensure proper documentation.
