We hate extra paperwork too so our new patient forms are available COMPLETELY ONLINE to make it less of a nightmare. Plus, if you knock them out before making your first appointment, we promise things will go a lot smoother.
All of our new patient forms may be filled out online. Once completed, you can just send them to ma@stlazarusfp.com or luis@stlazarusfp.com.
